Refresh imports and retire designs

Step 5 of 5 in Define credential designs and claims.

Refresh re-fetches the source with the stored ETag. When the source changed, it stores a new source snapshot and adds it to the design's sourceSnapshotIds; resolve then applies it. {snapshotId} comes from the design's sourceSnapshotIds. Delete a design only when no credential configuration or issuance template still references it.
